Account Access Agreement For NC DETECT
Thank you for your interest in NC DETECT! By logging into NC DETECT, you agree that you have read carefully and accept all of these Terms of Use, which constitute legally binding obligations.
The NC DETECT Web application is for users affiliated with health departments or a data provider, e.g. hospitals.
If you are not affiliated with a health department or a data provider,
please DO NOT COMPLETE THIS FORM.

Terms of Use
ID and Password
In order to access NC DETECT, you will need a User ID and Password. You are solely responsible in all respects for maintaining the confidentiality of your User ID and Password. The Account Holder agrees that this password will not be shared with anyone. You agree to notify NC DETECT immediately of any unauthorized use of your User ID or Password or any other breach of security regarding NC DETECT of which you are aware.
User Information
All User information provided to NC DETECT must be current, accurate, and complete. The Account Holder will notify NC DETECT immediately if his/her employment changes so that user's access can be adjusted. If NC DETECT at any time discovers any omission in the information you have provided, NC DETECT may, in its discretion, terminate your right to access and use NC DETECT.
Data Use
Access to the data is provided to Account Holders for the sole purpose of public health surveillance. The Account Holder understands and agrees that the data may not be disclosed to any third party, or used for any purpose other than public health surveillance, without the prior written permission of the NC Division of Public Health.
Disclaimers
NC DETECT shall not be liable for any claim, losses or damages of any kind arising from ACCOUNT HOLDER'S use of data. It is understood that no patent, copyright or other proprietary right is granted by this Agreement, and all rights to the NC DETECT data remain in the entities that supplied such data.
